Boarding Agreement Form

 Vaccinations are for the protection of your pet, we cannot make exceptions to vaccination requirements. If proof of vaccination is not on file, please provide us with current documentation of vaccine/medical history. WE REQUIRE:
Dogs: Distemper/Parvo (DHLPP), Rabies, and Kennel Cough
Cats: Feline Viral Rhinotracheitis, Calicivirus, Panleukopenia (FVRCP/Leuk.) and Rabies 

We make every effort possible to make your pet feel at ease while he/she is staying with us. Fort Payne Animal Hospital DOES NOT recommend bringing personal items from home as they may get lost in the laundry or soiled. Fort Payne Animal Hospital IS NOT responsible for lost or damaged personal items.

We offer a complimentary departure bath, including a nail trim, and anal sac expression, for all dogs staying 4 or more consecutive nights. If you do NOT want your pet to receive any of these services, please notify the representative at check-in. If your pet is here for less than 4 nights and you would like to purchase a basic bath, please let a representative know. 

1) I give my permission for the staff and veterinarians of Fort Payne Animal Hospital to administer care as needed while my pet is boarding.
2) I agree my pet shall not be allowed to leave the kennel until all fees and costs (as due) are paid to FPAH.
3) I understand I am solely responsible for any and all acts or behavior of my pet while it is in the care of FPAH, to include destruction of property to the Animal Hospital AND/OR the kennels to escape the facility, and injury to the staff and other pets in the care of hospital.
